CDBG Capital Improvement (CI) Project Forms and Packets Guidelines

The forms required for Community Development Block Grant (CDBG)-Capital Improvement construction projects are listed below.

Please note that prior to procuring or starting any CDBG-Capital Improvement construction project, the following forms must be completed and approved by Cook County's Department of Planning and Development (DPD) assigned project manager.

NOTE: Please do not fill out these forms in your browser. Before downloading the forms to your personal computer, please scroll down the page and review our downloading instructions if you are having difficulty. In addition, for each awarded CDBG project, an assigned DPD project manager will provide specific guidance.

Procurement and Construction Forms (Engineering, Construction Contractor, Architecture Services, Consultant, etc.)

NOTE: A publication is required for all projects $25K or over; projects under $25K must show proof of bid solicitation via regular mail, email, call list, etc.)

Form 4A1 (Professional Service Contracts - Labor Standards Compliance)

  • This form must be completed, signed, dated and returned to the assigned DPD Project Manager for approval prior to issuing a solicitation. Please be sure to attach the Request for Qualifications (RFQ), along with the Special Provisions for Federally Funded and Cook County CDBG-Funded Projects, the Davis-Bacon General Decision Wage Determination packet is not required for professional services. (Further instructions will be provided by the assigned DPD project manager.)

Form 4A (Construction Contracts - Labor Standards Compliance)

  • This form must be completed, signed, dated and returned to the assigned DPD Project Manager for approval prior to issuing a publication. Also, please be sure to attach the Request for Proposal (RFP) and bid specifications (prepared by an engineer, architect, consultant, etc.). It is imperative that all RFPs include the scope of work, specifications and Cook County’s Special Provisions for Federally Funded and Cook County CDBG-Funded Projects, along with the Davis-Bacon General Decision Wage Determination packet. It is optional to include the full Davis-Bacon Packet in with the RFP. (Further instructions will be provided by the assigned DPD project manager during the preconstruction meeting.)

Form 4B (Invitation to Bid)

  • Please use this “Invitation to Bid” template as a guide for preparing the bid publication and include required compliance language. Upon receiving the assigned DPD project manager’s signature approval on Form 4A, please proceed with the Bid Publication to include scheduled bid-opening date.

Special Provisions for Federally Funded and Cook County CDBG-Funded Projects

  • The Special Provisions for Federally Funded & Cook County CDBG-Funded Projects packet must be included as part of the bid proposal packet. The bid packet must also include the scope of work, specifications and most recent Davis-Bacon Wage Determination General Decision IL packet.

Form 4D1 (Bid/Contract Approval – Professional Services Only)

  • Prior to awarding professional services (engineer, architect, consultant, etc. – not to exceed $25K), this form along with the RFQ submittals must be reviewed and approved by the assigned DPD Project Manager. Please attach copies of the three lowest most responsible bid qualifications. Form 4D1 must be returned to the Subrecipient and/or its hired Engineer/Project Manager prior to awarding the selected professional service contractor.

Form 4D (Bid/Contract Approval – Construction Services Only)

  • Prior to awarding the selected construction contractor (contractors – $50K or over), the Form 4D form must be completed and submitted, along with the three lowest most responsible bid proposals, to the assigned DPD Project Manager prior to awarding the construction contractor. Form 4D and the three lowest responsible bid proposals must be reviewed and approved by the assigned DPD project manager as required.

Form 4CO (Change Order for Construction Only)

  • Change orders are the exception and not the rule. All changes must be reviewed and approved by the
    Subrecipient and/or their hired Engineer/Project Manager, Contractor, assigned DPD Project Manager, Inspector (if needed), and Cook County DPD upper management manager. The change order must not exceed the original CDBG project award. (The Form 4CO Change Order form will be made available upon request; please contact the assigned DPD Project Manager.)

Form 4E (Project Completion/Closeout)

  • When construction is 100-percent complete, the Subrecipient’s Project Manager and/or its hired Engineer must complete and submit the Form 4E project closeout form along with all required supporting documentation, as noted on the project closeout form.

Davis-Bacon Construction Packet

  • Most of the required compliance closeout forms are located within Cook County Davis-Bacon Construction Packet. The information is reviewed in detail during the preconstruction meetings, all forms must be completed and returned with Form 4E after the project is 100-percent completed. No payments will be disbursed until all required forms and compliance documentation have been received and approved.

  1. PLEASE DO NOT FILL OUT OUR FORMS IN YOUR BROWSER.  In order to use the fillable forms, you must have Adobe 9.0 Reader or above on your computer. If you do not have Adobe Reader 9.0 or above, please visit http://get.adobe.com/reader/ and download the latest version of Adobe Reader onto your computer for FREE.
  2. Download the blank forms to your computer and open in your Adobe Reader 9.0 or above.
  3. Fill out each form.
  4. Once you complete filling out each form, please use the SAVE feature and save the completed form with the new name and keep your electronic copy on your computer. (In order to save the completed pdf, it must have a different name than the name of the blank pdf you downloaded.)
  5. Return your completed forms to your assigned DPD staff person.
